• Current manufacturer and part number: If you are evaluating a replacement (optional).
• Application: What liquid or process you are filtering, and your filtration goal.
• Estimated annual usage: Approximate number of elements or filters your facility uses (if known).
• Shipping destination: City, province/state, and country (no full street address needed for the initial request).

You can also mention your preferred micron rating, filter length, end-cap configuration, and when you would like to begin testing if relevant.
